therefore, nurses do not need to delegate topical medication administration to PSWs.<\/li>\n<li>CSS should assess the client\u2019s current skin condition.<\/li>\n<li>PSWs can apply topical medications on intact skin only and cannot provide treatment to broken skin or a wound.<\/li>\n<li>The CSS must advise the PSW of an appropriate amount of cream\/ointment to be used if not indicated on the prescription.<\/li>\n<\/ul>\n<h3>PSW Considerations:<\/h3>\n<p>Report to the office at 416-635-2860 if:<\/p>\n<ul>\n<li>The client has signs of:\n<ul>\n<li>Broken skin<\/li>\n<li>Pain<\/li>\n<li>Tender to touch<\/li>\n<li>Itchiness<\/li>\n<\/ul>\n<\/li>\n<li>The client\u2019s skin condition is getting worse (affected area is getting larger) and the treatment is not effective<\/li>\n<li>The cream is not in a container with the appropriate pharmacy labels, or the cream is not in the original container<\/li>\n<li>The prescribed cream\/ointment is running low or not available for use<\/li>\n<\/ul>\n<h3>Required Supplies:<\/h3>\n<ul>\n<li>Topical ointment<\/li>\n<li>Gloves<\/li>\n<li>Soap and water (to clean the affected area)<\/li>\n<\/ul>\n[\/vc_column_text][\/vc_column][\/vc_row][vc_row type=&#8221;in_container&#8221; bg_image_animation=&#8221;none&#8221; border_type=&#8221;simple&#8221; column_border_width=&#8221;none&#8221; column_border_style=&#8221;solid&#8221;][vc_column_text]\n<h3>Procedure for Applying Ointments (Topical Route):<img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-8249 size-medium\" src=\"https:\/\/psw.circleofcare.com\/wp-content\/uploads\/2024\/01\/close-up-wounded-knee-with-ointment-300x169.jpg\" alt=\"\" width=\"300\" height=\"169\" srcset=\"https:\/\/psw.circleofcare.com\/wp-content\/uploads\/2024\/01\/close-up-wounded-knee-with-ointment-300x169.jpg 300w, https:\/\/psw.circleofcare.com\/wp-content\/uploads\/2024\/01\/close-up-wounded-knee-with-ointment-768x432.jpg 768w, https:\/\/psw.circleofcare.com\/wp-content\/uploads\/2024\/01\/close-up-wounded-knee-with-ointment.jpg 1000w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/h3>\n<ol>\n<li>Before beginning, assist the client with a sponge bath or shower as per the care plan, or cleanse the area where the topical medication is to be applied.<\/li>\n<li>Review the delegation notes in the client\u2019s care plan via your mobile device.<\/li>\n<li>Explain the procedure to the client.<\/li>\n<li>Perform hand hygiene.<\/li>\n<li>Gather the ointment and materials that are needed.<\/li>\n<li>Check the label of the ointment for the expiration date.<br \/>\n\u2022 Note: Do not use the medication if it is past the expiration date.<\/li>\n<li>Open the ointment tube or container.<\/li>\n<li>Place the cap of the ointment tube or container upside down on a table top to prevent contamination of the inner aspect of the cap.<\/li>\n<li>Put on gloves.<\/li>\n<li>Apply the topical medication onto the ordered area(s) using gloved hands, a tongue depressor, a cotton tipped applicator, or sterile gauze.<\/li>\n<li>Apply the topical medication in long and even strokes, following the direction of hair growth.<\/li>\n<li>Remove gloves and wash hands thoroughly.<\/li>\n<li>Put away all the supplies used.<\/li>\n<\/ol>\n[\/vc_column_text][\/vc_column][\/vc_row][vc_row type=&#8221;in_container&#8221;
