Every year, Circle of Care is required to update its list of employees whose family members receive Circle of Care services. This includes, but is not limited to: Homemaking and Personal Support, Holocaust Survivor Services, Meals on Wheels, iRIDE and Adult Day Program.<\/p>\n

This is part of our ongoing commitment to uphold privacy legislation (PHIPPA) and also, is a part of our contractual obligations with our funders.<\/p>\n

If you have a family<\/u> member who is receiving Circle of Care services, please respond to Dara Zarnett at dzarnett@circleofcare.com<\/a><\/u>\u00a0with the following information by Friday, January 17th<\/b>\u00a0:\u00a0<\/span><\/p>\n

  1. Your name<\/li>\n
  2. Name\u00a0of\u00a0the relative receiving Circle of Care services<\/li>\n
  3. Your relationship to the client<\/li>\n
  4. The type\u00a0of\u00a0service\/program<\/li>\n<\/ol>\n

    Please notify Dara, even if you have previously done so in the past.\u00a0 Not disclosing this could result in the loss of services for your relative.<\/p>\n
