If you are looking to take time off <strong>more than 10 days<\/strong>, please request this time off using our <strong>Leave of Absence\/Time Off Request form<\/strong> and select <strong>\u201cExtending Vacation Time\u201d<\/strong> in the <strong>Reason for Leave section<\/strong> on the <strong><a href=\"https:\/\/circleofcare.formstack.com\/forms\/loa_time_off_request\">form<\/a>.<\/strong><\/p>\n<p><strong><span style=\"text-decoration: underline;\">Note:<\/span> You do not need to take a leave of absence to cover vacation time as long as you have sufficient vacation entitlements available.<\/strong><\/p>\n<ul>\n<li>Once your request has been submitted, it will be reviewed by your CSS\/SC and\/or Human Resources.<\/li>\n<li>Requests will be approved or declined based upon seniority and needs of the organization.<\/li>\n<li>Vacation requests can be submitted as far in advance of their vacation as possible, however, during desired vacation times (Christmas period and July\/August) requests shall be submitted no less than eight (8) weeks prior to the intended vacation.<\/li>\n<li>The vacation period is from January 1 to December 31 in each year. <strong>Please refer to the chart below:<\/strong><br \/>\n<table style=\"height: 185px;\" width=\"448\">\n<tbody>\n<tr>\n<td width=\"194\"><strong>Vacation Quarter<\/strong><\/td>\n<td width=\"231\"><strong>Last Day to Submit Requests<\/strong><\/td>\n<td width=\"151\"><strong>\u00a0Approval No Later<\/strong><\/td>\n<\/tr>\n<tr>\n<td width=\"194\">January to March<\/td>\n<td width=\"231\">November 1<\/td>\n<td width=\"151\">November 22<\/td>\n<\/tr>\n<tr>\n<td width=\"194\">April to June<\/td>\n<td width=\"231\">February 1<\/td>\n<td width=\"151\">February 22<\/td>\n<\/tr>\n<tr>\n<td width=\"194\">July to September<\/td>\n<td width=\"231\">May 1<\/td>\n<td width=\"151\">May 22<\/td>\n<\/tr>\n<tr>\n<td width=\"194\">October to December<\/td>\n<td width=\"231\">August 1<\/td>\n<td width=\"151\">August 22<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/li>\n<li>You will be notified of the <strong>approval or denial of your vacation request no later than three (3) weeks<\/strong> from the cutoff date of receipt of the request by the employee\u2019s supervisor. You can check your vacation requests through the PSW App on your Circle of Care mobile phone to see if they have been approved or declined.\u00a0 <strong><span style=\"color: #ff0000;\">You are responsible to verify whether your request is approved or declined.<\/span><br \/>\n<\/strong><\/li>\n<li>Vacation <strong>shall not be accumulated from year to year<\/strong> and shall not be carried over the next vacation year.<\/li>\n<li>Employees will be given preference with respect to vacation periods in accordance with seniority, subject to the employer\u2019s requirements as to sufficient availability of staff to meet the needs of the employer\u2019s clients.<\/li>\n<\/ul>\n[\/vc_column_text][\/vc_column][\/vc_row][vc_row type=&#8221;in_container&#8221;
