To participate in this program, please read the following steps below:<\/p>\n

  1. Complete the form below. (Note: If Human Resources is not notified of the referral prior to the start date of the candidate or within 30 days of the start date, the referral bonus will not be approved<\/strong>. Forms received outside of this timeframe will not be reviewed.)<\/li>\n
  2. HR will consider the potential applicant and contact the individual directly to proceed with an interview if the candidate\u2019s work experience, skills and education match the job requirements of an available position.<\/li>\n
  3. If your candidate is hired, Human Resources will notify you on a successful referral.<\/li>\n
  4. Payment will be paid in two separate amounts.\u00a0 The first payment will be paid after the referred candidate has successfully completed their probationary period. The second half of the payment will be paid after the candidate completes one full year of service with Circle of Care.<\/li>\n
  5. You must be actively employed by Circle of Care to remain eligible for the award.<\/li>\n
  6. Referrers who are eligible for payout will receive the amount of the payout, less applicable taxes.<\/li>\n<\/ol>\n
