{"id":4484,"date":"2021-09-09T09:00:39","date_gmt":"2021-09-09T09:00:39","guid":{"rendered":"https:\/\/psw.circleofcare.com\/?p=4484"},"modified":"2022-04-21T20:45:12","modified_gmt":"2022-04-21T20:45:12","slug":"reminder-download-send-in-your-covid-19-vaccine-receipt","status":"publish","type":"post","link":"https:\/\/psw.circleofcare.com\/index.php\/2021\/09\/09\/reminder-download-send-in-your-covid-19-vaccine-receipt\/","title":{"rendered":"Updated Deadline: Download &#038; Send in your COVID-19 Vaccine Receipt"},"content":{"rendered":"<h4>Reminder: Download &amp; Send in your COVID-19 Vaccine Receipt<\/h4>\n<p>In compliance with Circle of Care\u2019s Mandatory COVID-19 Vaccination Policy, all employees are required to provide proof of full vaccination against COVID-19 by submitting a copy of the receipt of their second dose of COVID-19 vaccine to <span style=\"text-decoration: underline;\">HRDept@circleofcare.com <\/span><b>by OCTOBER 15, 2021<\/b><b>.<\/b><\/p>\n<p>COVID-19 vaccine receipts are available to be downloaded after each dose is administered in Ontario and can be downloaded through the <a href=\"https:\/\/covid-19.ontario.ca\/covid-19-vaccine-booking-support#vaccination-receipts\">online provincial portal<\/a> where you will be prompted to input your postal code, date of birth, and details from your green OHIP card.<\/p>\n<p>For any employees who have a red and white OHIP card, you can call <strong>1-833-943-3900<\/strong> to request a copy be emailed to you.<\/p>\n<p>If you still need your first or second shot, vaccination is more widely available now.
