{"id":4513,"date":"2021-09-24T15:49:25","date_gmt":"2021-09-24T15:49:25","guid":{"rendered":"https:\/\/psw.circleofcare.com\/?p=4513"},"modified":"2021-09-24T15:49:25","modified_gmt":"2021-09-24T15:49:25","slug":"mandatory-covid-19-vaccination-policy-update","status":"publish","type":"post","link":"https:\/\/psw.circleofcare.com\/index.php\/2021\/09\/24\/mandatory-covid-19-vaccination-policy-update\/","title":{"rendered":"Mandatory COVID-19 Vaccination Policy Update"},"content":{"rendered":"<p>Circle of Care\u2019s <a href=\"https:\/\/drive.google.com\/file\/d\/1ddFJ8DuwnOc40fEpOvwE6PUdhNKXKip0\/view?usp=sharing\">Mandatory COVID-19 Vaccination Policy<\/a> has been updated in response to Directive #6 from the Ontario Chief Medical Officer of Health (CMOH) to now expand eligible vaccines to include all approved COVID-19 vaccines by the World Health Organization (WHO).<\/p>\n<p>A reminder that in compliance with Circle of Care\u2019s Mandatory COVID-19 Vaccination Policy, all employees are required to provide proof of full vaccination against COVID-19 by submitting a copy of the receipt of their second dose of COVID-19 vaccine to HRDept@circleofcare.com <strong><span style=\"text-decoration: underline;\">by OCTOBER 15, 2021<\/span>.<\/strong><\/p>\n<p>COVID-19 vaccine receipts are available to be downloaded after each dose is administered in Ontario and can be <a href=\"https:\/\/covid-19.ontario.ca\/covid-19-vaccine-booking-support#vaccination-receipts\">downloaded through the online provincial portal<\/a> where you will be prompted to input your postal code, date of birth, and details from your green OHIP card.<\/p>\n<p>For any employees who have a red and white OHIP card, you can call 1-833-943-3900 to request a copy be emailed to you.<\/p>\n<p>If you still need your first or second shot, vaccination is more widely available now.
