Circle of Care\u2019s Mandatory COVID-19 Vaccination Policy has been updated in response to Directive #6 from the Ontario Chief Medical Officer of Health (CMOH) to now include measures for all employees for non-compliance with this policy. Failure to adhere to this policy will result in progressive discipline, up to and including dismissal. The updated policy is available on the PSW portal.<\/p>\n

A reminder that In compliance with Circle of Care\u2019s Mandatory COVID-19 Vaccination Policy, all employees are required to provide proof of full vaccination against COVID-19 by submitting a copy of the receipt of their second dose of COVID-19 vaccine to\u00a0HRDept@circleofcare.com<\/a> by OCTOBER 15, 2021<\/strong><\/u>.<\/strong><\/p>\n <\/p>\n COVID-19 vaccine receipts are available to be downloaded after each dose is administered in Ontario and can be downloaded through the\u00a0online provincial portal<\/a>\u00a0(https:\/\/covid-19.ontario.ca\/covid-19-vaccine-booking-support#vaccination-receipts<\/a>)\u00a0where you will be prompted to input your postal code, date of birth, and details from your green OHIP card.<\/p>\n <\/p>\n For any employees who have a red and white OHIP card, you can call\u00a01-833-943-3900<\/strong>\u00a0to request a copy be emailed to you.<\/p>\n <\/p>\n If you still need your first or second shot, vaccination is more widely available now.
