{"id":9865,"date":"2024-11-25T13:19:08","date_gmt":"2024-11-25T13:19:08","guid":{"rendered":"https:\/\/psw.circleofcare.com\/?p=9865"},"modified":"2024-11-25T13:19:08","modified_gmt":"2024-11-25T13:19:08","slug":"new-program-launch-sinai-health-to-home","status":"publish","type":"post","link":"https:\/\/psw.circleofcare.com\/index.php\/2024\/11\/25\/new-program-launch-sinai-health-to-home\/","title":{"rendered":"New Program Launch: Sinai Health to Home"},"content":{"rendered":"<div><b>New Program Launch \u2013 Sinai Health to Home<\/b><\/div>\n<div><\/div>\n<div>We are excited to let you know that Sinai Health has partnered with Circle of Care to launch Sinai Health to Home, as part of Ontario Health\u2019s plan to expand provincial Hospital to Home programs.<\/div>\n<div><\/div>\n<div>Sinai Health to Home program is designed to support patients\u2019 transition from hospital back into their homes by providing them with time-limited supports tailored to their specific needs. There is a focus on care coordination and improving the patient and caregiver experience during the hospital to home transition.<\/div>\n<div><\/div>\n<div>As the service provider organization, Circle of Care will provide and manage the services patients receive in their home. Care plans will be created to meet each patient\u2019s individual home and community support needs and regularly reviewed to ensure services continue to match needs to a maximum of 16 weeks.<\/div>\n<div><\/div>\n<div>This new model of home care is different from the \u2018traditional\u2019 model in that hospitals and service providers work directly with each other to support patients.\u00a0 It is a transitional program that aims to support people being appropriately discharged to the community with the services they need, with the goal of successfully transitioning them off the program within a defined time period. This allows for seamless care, better communication and less fragmentation.<\/div>\n<div><\/div>\n<div>We are proud of this partnership, and we know it will make a big difference in people\u2019s lives.
