Every year, Circle of Care needs to know which employees have family members using our services, such as Homemaking and Personal Support, Holocaust Survivor Services, Meals on Wheels, iRide, Adult Day Program, and others.

This helps us follow privacy laws (PHIPPA) and meet our funding requirements.

If a family member of yours is receiving Circle of Care services, please send the following information to Dara Zarnett at dzarnett@circleofcare.com by Friday, January 16th:

  • Your name

  • Name of your relative receiving services
  • Your relationship to them

  • The type of service or program they use

Please provide this information even if you’ve shared it before. Failing to disclose could affect your relative’s access to services.
