Conflict of Interest Advisory

Every year Circle of Care updates its list of employees whose family members receive Circle of Care services. This is for any client that receives any service (LHIN, Claims Conference, private pay etc.), and is a requirement from our funders.

If you have a family member who receives services from Circle of Care, you must let us know. Please email your name, client ID of family member and your relationship to quality@circleofcare.com by February 5, 2021. Not responding to this request could result in the loss of service for family members.
