Do you need a copy of your vaccine receipt?
COVID-19 vaccine receipts are available to download or print after each dose administered (first or second doses) in Ontario, regardless of the type of vaccination clinic where the dose was administered (e.g. hospital, City-run, primary care, pharmacy, or pop-up).
How do I access my vaccine receipts?
A green photo health card, birth date, and postal code are required to download receipts from the online provincial portal. People with red and white health cards or without Ontario health cards can call the Provincial Vaccine Booking Line at 1-833-943-3900, and ask an agent to email a copy of the receipt.
What will I need my vaccine receipt for?
You may potentially need to have proof of vaccination handy in the near future when the rules for self-isolation for fully vaccinated healthcare workers changes. You may also need proof of vaccination for personal use, such as for international travel.
