Circle of Care’s Mandatory COVID-19 Vaccination Policy has been updated to further clarify approved exemptions. A Rapid Antigen Test Policy is available now for further guidelines on testing to those who have approved exemptions. Failure to adhere to these policies will result in progressive discipline, up to and including dismissal. The updated policies are available on the PSW portal.
All employees, volunteers, and students will be required to provide one of the following:
1. Proof of full vaccination against COVID-19 by submitting a copy of the receipt (or similar) of second dose of approved vaccine; or
2. Written proof of a specific medical reason for exemption, provided by either a physician or nurse practitioner that sets out that the person cannot be vaccinated against COVID-19; and the effective time period for the medical reason (i.e., permanent or time-limited).
All individuals who are unvaccinated due to an approved exemption or who are awaiting their second dose of a two-dose COVID-19 vaccine are required to be subject to regular Rapid Antigen Testing (RAT) at a minimum of two (2) times per workweek or at intervals as deemed appropriate by the Agency and demonstrate a negative test result. A reminder that in compliance with Circle of Care’s Mandatory COVID-19 Vaccination Policy, all employees are required to provide proof of full vaccination against COVID-19 by submitting a copy of the receipt of their second dose of COVID-19 vaccine to HRDept@circleofcare.com
COVID-19 vaccine receipts are available to be downloaded after each dose is administered in Ontario and can be downloaded through the online provincial portal (https://covid-19.ontario.ca/covid-19-vaccine-booking-support#vaccination-receipts) where you will be prompted to input your postal code, date of birth, and details from your green OHIP card.
For any employees who have a red and white OHIP card, you can call 1-833-943-3900 to request a copy be emailed to you.
