In late 2021, you had the opportunity to share feedback with us through the Accreditation Canada’s Client Safety Culture Survey. This confidential survey helps us measure our success at promoting and encouraging a culture of client safety. This includes how well we support a blame-free way of thinking that encourages learning from errors and mistakes.
Survey says…
276 employees took the time to share their feedback, and overall results were very positive!
We scored a green flag (positive result) on 17 out of 23 questions, which highlight the following themes:
- Safety decisions are made by qualified people.
- Senior management promotes client safety.
- There is follow-up around risk and client safety events.
- Staff input and feedback is accounted for when addressing and following up on client safety events.
Room for Improvement
- Six questions represent opportunities for improvement (red or yellow flag), including a focus on fostering a “blame-free” culture where risks and errors can be reported without feeling ashamed or fearful.
- People reported feeling that if they made a serious error, co-workers would lose respect for them, they could lose their jobs, and/or their managers would think they are incompetent.
There has been some improvement in the results since 2017; three of questions that were coded red in 2017 moved to yellow in 2021.
These results have been shared with Managers who are working with teams to develop appropriate action plans. Stay tuned for more information about blame-free culture in our next newsletter!
