Disclosing Client Safety Incidents (ROP)

Having a documented and coordinated approach to disclosing client safety incidents to clients and family members is a Required Organizational Practice for our upcoming Accreditation. The goal of the disclosure plan should be to promote communication and ensure a supportive response.

When are safety incidents disclosed?

Whenever a client suffers harm, we are required to communicate to the client about the harm and the events that led to the harm. Incidents must be disclosed if there is current harm related to the client, or risk of potential or future harm. If there is no harm or the situation was a near miss, the Most Responsible Person and Manager can determine if the event should be disclosed.

What takes place during disclosure?

During the disclosure, Circle of Care will provide an apology to the client and/or family member, as well as emotional and psychological support for clients, caregivers and staff involved in the adverse event. During disclosures, the Most Responsible Person will avoid blame and speculation, discussing only the agreed upon facts. There are also guidelines for timing for communication and documentation outlined in the policy.
