New Program Launch – Sinai Health to Home
We are excited to let you know that Sinai Health has partnered with Circle of Care to launch Sinai Health to Home, as part of Ontario Health’s plan to expand provincial Hospital to Home programs.
Sinai Health to Home program is designed to support patients’ transition from hospital back into their homes by providing them with time-limited supports tailored to their specific needs. There is a focus on care coordination and improving the patient and caregiver experience during the hospital to home transition.
As the service provider organization, Circle of Care will provide and manage the services patients receive in their home. Care plans will be created to meet each patient’s individual home and community support needs and regularly reviewed to ensure services continue to match needs to a maximum of 16 weeks.
This new model of home care is different from the ‘traditional’ model in that hospitals and service providers work directly with each other to support patients. It is a transitional program that aims to support people being appropriately discharged to the community with the services they need, with the goal of successfully transitioning them off the program within a defined time period. This allows for seamless care, better communication and less fragmentation.
