Every year, Circle of Care is required to update its list of employees whose family members receive Circle of Care services. This includes, but is not limited to: Homemaking and Personal Support, Holocaust Survivor Services, Meals on Wheels, iRIDE and Adult Day Program.

This is part of our ongoing commitment to uphold privacy legislation (PHIPPA) and also, is a part of our contractual obligations with our funders.

If you have a family member who is receiving Circle of Care services, please respond to Dara Zarnett at dzarnett@circleofcare.com with the following information by Friday, January 17th : 

  1. Your name
  2. Name of the relative receiving Circle of Care services
  3. Your relationship to the client
  4. The type of service/program

Please notify Dara, even if you have previously done so in the past.  Not disclosing this could result in the loss of services for your relative.
