Circle of Care’s Mandatory COVID-19 Vaccination Policy has been updated in response to Directive #6 from the Ontario Chief Medical Officer of Health (CMOH) to now include measures for all employees for non-compliance with this policy. Failure to adhere to this policy will result in progressive discipline, up to and including dismissal. The updated policy is available on the PSW portal.
A reminder that In compliance with Circle of Care’s Mandatory COVID-19 Vaccination Policy, all employees are required to provide proof of full vaccination against COVID-19 by submitting a copy of the receipt of their second dose of COVID-19 vaccine to HRDept@circleofcare.com by OCTOBER 15, 2021.
COVID-19 vaccine receipts are available to be downloaded after each dose is administered in Ontario and can be downloaded through the online provincial portal (https://covid-19.ontario.ca/covid-19-vaccine-booking-support#vaccination-receipts) where you will be prompted to input your postal code, date of birth, and details from your green OHIP card.
For any employees who have a red and white OHIP card, you can call 1-833-943-3900 to request a copy be emailed to you.
