Transdermal Patches: Medicated adhesive patch placed on the client’s skin to deliver medication through the skin into the bloodstream.
CSS Considerations
- Transdermal patches do not include fentanyl or any other narcotic patches.
- The client must be cognitively intact and able to self-direct their care.
PSW Considerations
- Sites where patches can be applied:
- Upper outer arm
- Upper chest
- Upper back
- Side of chest
- Do not allow the client to wear two patches at once
- The PSW should report to the office at 416-635-2860 if the patch was not applied or removed as scheduled
- The client should indicate where the patch is located to assist the PSW with removal
- Do not apply the transdermal patch and report to the office immediately if the client has any of the following symptoms:
- Rash
- Hives
- Itching
- Difficulty breathing or swallowing

- Transdermal patch
- Pen/marker
Procedure for Applying Transdermal Patches
Documentation by PSW
- Before applying the patch, remove the patch from the packaging. Write down the date and your initials on the patch.
- Sign off the MAR sheet when the patch is applied or removed.
Application of Transdermal Patch
- Wash hands thoroughly with soap. Put on gloves.
- Hold the patch so that the plastic backing is facing you.
- Bend the sides of the patch away from you and then toward you until you hear a snap.
- Peel off one side of the plastic backing.
- Apply the patch to a clean and dry area that is free of hair, scars, cuts, or irritation. The patch should be applied to a different site than the previous patch.
- Apply the sticky half of the patch to the client’s skin on the area directed. Use the other side of the patch as a handle.
- Press the sticky side of the patch against the skin and smooth it down.
- Fold back the other side of the patch. Remove the plastic backing and pull the patch across the skin.
- Remove gloves and perform hand hygiene.
Removal of Transdermal Patch
- Wash hands thoroughly with soap. Put on gloves.
- Press down on the center of the patch to lift the edges away from the skin.
- Hold the edge gently and slowly peel the patch away from the client’s skin.
- Fold the patch in half with the sticky sides pressed together and dispose of the patch in the garbage.
- Remove gloves and perform hand hygiene.
