Risk Management
The health and safety of our clients, employees and volunteers is our top priority. Additionally, having a structured process to identify and analyze actual and potential risks is an Accreditation standard.
We have a number of systems in place to manage, communicate and learn from all risk events:
Client Safety/Risk Events
These are unforeseen events that give rise to or have the potential to increase danger or injury related to service delivery.
Examples include client/employee/volunteer injury or fall, missed care, medication error, alleged/actual elder abuse, alleged/actual theft, accidental property damage, unsafe living conditions and alleged/actual abuse, harassment, or hate motivated behaviour towards workers.
Tracking
All client safety risks are entered in EasyCare as a Risk and Safety Event case note. Risk events flag to the Risk and Safety Task Console, which allows each risk event to be sent as a Task Console to the most responsible person, usually the Client Service Supervisor or Case Manager. On a weekly basis, a report is generated which captures all Risk and Safety case notes as well as other notes that have been identified as risk events through keyword flagging. This report is reviewed by the Quality Team and the information is converted to data, which is reported on a monthly basis through our Heat Map of Key Metrics.
Reporting
Client safety risks are reported to the senior leadership team, our Board of Directors, our Quality Subcommittee and our HCCSS funders to ensure we hold ourselves accountable. See our Event Reporting policy for more details.
Disclosure
Circle of Care is committed to disclosing adverse risk events to clients, families, and caregivers. In an effort to continuously improve our processes and ensure transparency, our Disclosure Policy outlines the necessary steps for managing, disclosing, reporting and analyzing these safety events.
Enterprise Risk Management
Actively monitoring our organizational risks is another way we ensure safety across Circle of Care. By keeping an eye on corporate risks (e.g. financial, IT, security, regulatory, liability, etc.) we can identify and address any potential threats.
